If you plan to pick up your student at away games and/or competition the attached form must be used; a new one is required for each event.
The attached form needs to be turned in to a staff member or chaperone prior to leaving Mars. Students are expected to stay for the duration of the game and/or competition unless other arrangements are made with the staff.
